INTERNSHIP LEARNING AND PROCEADURES PERFORMED

 Hello,I am M.Vineela from 2k18 batch.

In this blog I would like to share my experiences and learning procedures in  medicine department .


I am posted from October 1 to November 30 in medicine department. 

For the first 15 days,I.e from 1 to 15  I was posted in psychiatry patient.

Case history

Chief complaints 

According to patient - Pain in scrotum since 6 months

Belief that everyone is talking about him since 6 months 

According to informant -ABNORMAL behavior in form of irritability ,frequent anger outbursts,suspiciousness on family members and strangers

HOPI-He was apparently asymptomatic 6 months ago when all of sudden he came home from his workplace at Hyderabad and said to  his parents that he lost his job and fell in love with girl there who didn't feel the same way where he ended up getting hurt in scrotum .From then he has been very highly irritable ,gets angry and goes to hit family members and always has suspicion that he is being talked about and someone is trying to harm him 

Substance history- H/O alcohol consumption present, stopped since 5 months 

H/O cannabis use twice 2 years ago

H/O tobacco use 3 years ago ,currently smoking 3 to 4 cigarettes per day 


DIAGNOSIS -PARANOID SCHIZOPHRENIA 

TREATMENT-

*Counselling and psychoeducation

*TAB RIPERIODONE 2MG +THG 2MG

*TAB DIVALPROATE 500MG 

*TAB CLOZAPINE 25MG 

*NICOTINE GUMS 



For the next 15 days I completed my peripheral posting I.e nephro,icu and ward duties

**I did central line suturing for 2 patients 

*InternalJugular 

*&Femoral 

I've seen some cases of nephrology .one case among the following is 

Case**

Patient came with C/O burning micturition since 1 month 

Decreased urine output since 15 days 

SOB since 10 days 

Fever 20 days ago,relevied with medication 

Orthopnea is present.No PND

He went to other hospital and 4 sessions of hemodialysis were done.He came to our hospital for further treatment.

During my ICU i monitored vitals for critically ill patients.

 I did one cpr with my co interns and pg
